Your twelve year old dog has started waking you at three in the morning for no apparent reason. Or she stands in the middle of the kitchen staring at the wall. Or he walked straight past his food bowl yesterday and seemed genuinely confused about where it was. Or she had an accident in the house for the first time in a decade.

You probably told yourself it was just old age. You are not entirely wrong — it is an age-related condition. But “just old age” undersells what is actually happening and, more importantly, suggests there is nothing to be done. There is.

Canine Cognitive Dysfunction Syndrome — CDS, sometimes called canine dementia — is a degenerative brain condition that affects senior dogs in ways that closely parallel Alzheimer’s disease in humans. It is estimated to affect more than 30 million dogs in the USA alone and up to 60% of older dogs overall. (PMC, Butyrylcholinesterase Study)

Most cases go undiagnosed. Most owners attribute the symptoms to normal aging. Most dogs with CDS never receive treatment that could meaningfully improve their quality of life. (Frontiers in Veterinary Science, 2025)

This article is a guide to recognizing what is happening, understanding what is causing it, and knowing what can actually help.

What Is Canine Cognitive Dysfunction

CDS is a neurodegenerative disorder — meaning the brain gradually deteriorates over time rather than being damaged by a single event. The pathological changes in a dog’s brain with CDS are strikingly similar to those seen in human Alzheimer’s disease: accumulation of beta-amyloid protein forming plaques that disrupt synaptic function, tau protein phosphorylation, neuronal loss, and neuroinflammation. These changes reduce the brain’s ability to process information, form and retrieve memories, regulate sleep, and manage the behavioral responses that a dog uses to navigate its environment. The dog that appears confused, disoriented, or unusually anxious is not being difficult or suddenly badly behaved. The brain that has always reliably told it where the food bowl is and when to go outside is no longer doing so reliably.

Unlike many canine health conditions, CDS does not appear to be breed-specific. No single breed is significantly more affected than others. Spayed female dogs are the most likely to develop the disease. (Embrace Pet Insurance, 2025)

The Ages Most Commonly Affected

CDS generally begins after age nine, though early signs are often subtle enough to be missed at this stage. The prevalence increases significantly with age. (VIN Veterinary Partner, 2024)

One study found that 28% of dogs aged 11 to 12 years show at least one sign of CDS. By age 15 and older, that figure rises to 68%. (AKC, Dr. Denenberg, 2026)

In practical terms this means that if your dog is over eleven, there is roughly a one in four chance they are already showing some degree of cognitive decline. If they are fifteen or older, the odds are higher than not. The condition is not rare. It is common. The gap between how common it is and how often it is recognized and treated is the problem this article is trying to address.

Larger dogs tend to age faster than smaller dogs, which means larger breeds may show signs of CDS at younger ages than the general figures suggest. A large breed dog at ten may be at a similar cognitive aging stage as a small breed dog at thirteen or fourteen.

Symptoms You May Be Seeing But Not Recognizing

The challenge with CDS is that many of its symptoms look like other things. Some look like normal aging. Some look like behavioral problems. Some look like physical health issues. The DISHAA framework — used by veterinary professionals to assess CDS — covers the primary symptom categories.

Disorientation

Your dog gets lost in familiar spaces. They walk into a room and stand still, appearing uncertain of where they are or why they came in. They get stuck in corners and cannot figure out how to back out or turn around. They fail to recognize familiar people or other pets in the household. They stare blankly at walls or into space for extended periods. They go to the wrong side of a door — pushing against the hinge side rather than the opening side.

These are not personality quirks or signs of stubbornness. They are signs of spatial disorientation produced by neurological change.

Interaction Changes

A dog that has always been affectionate seeks less contact, or seeks more contact than before. A dog that was socially engaged with family members seems to withdraw or becomes less interested in interactions. Conversely, a previously independent dog may become unusually clingy and anxious when separated from their person. Reduced response to their name or familiar commands — not because of hearing loss but because of cognitive processing change — is another common sign.

Sleep-Wake Cycle Disturbances

This is one of the most disruptive symptoms for the people who live with an affected dog. The dog that used to sleep through the night begins waking at two or three in the morning — restless, sometimes vocal, appearing confused. They may sleep more during the day and less at night, reversing the normal sleep pattern. Nighttime vocalization — howling or whining at night without an obvious trigger — is a classic CDS symptom that is frequently misattributed to pain or anxiety rather than cognitive dysfunction.

House Soiling

A previously housetrained dog begins having accidents indoors. This is not a regression or a behavioral problem — it is the dog failing to remember the learned behavior of going outside, or failing to associate the physical sensation of needing to eliminate with the action of going to the door. This symptom is often deeply upsetting to owners who interpret it as the dog “knowing better.” The dog does not know better in the way they once did. That knowledge is being eroded. Activity Level Changes

Reduced exploration, reduced interest in play, reduced engagement with toys or activities the dog previously enjoyed. Some dogs show increased restless pacing alongside reduced purposeful activity — moving around a lot without apparent goal or direction. Reduced self-grooming is also documented.

Anxiety

Increased general anxiety, particularly separation anxiety, is a common CDS symptom. The dog may become distressed when separated from their person in ways they never were before. Unfamiliar environments or changes to routine that were previously handled without difficulty may produce significant distress. (Senior Dog Revolution, 2024)

Diagnosis

There is no single definitive test for CDS. Diagnosis involves a thorough veterinary evaluation that rules out other conditions whose symptoms overlap with CDS — including hypothyroidism, pain, urinary tract infection, vision or hearing loss, hypertension, liver disease, and brain tumors, all of which can produce behavioral changes in older dogs. Your vet will review the dog’s full history, conduct a physical and neurological examination, and likely run bloodwork and urinalysis. In some cases CT or MRI imaging of the brain may be recommended. The DISHAA questionnaire — which covers the symptom categories above — is a standardized tool that helps veterinarians assess the degree of cognitive dysfunction.

If you are concerned about your senior dog, go in with specific examples. When did the behavior start? How often does it happen? Has it gotten worse? The more specific information you can provide, the more useful the evaluation will be.

Medications That Can Help

Selegiline (Anipryl)

Selegiline — sold under the brand name Anipryl — is the only medication currently approved by the FDA specifically for the treatment of CDS in dogs. It is an MAO-B inhibitor that works by increasing dopamine levels in the brain, improving neuronal communication, and providing some neuroprotective effects. (VIN Veterinary Partner, 2024)

Clinical studies have shown that approximately 30% of dogs show significant improvement on selegiline, with another 30% showing some improvement. It is most effective in the early to moderate stages of CDS and is generally well tolerated. It requires a prescription and is administered daily. (Frontiers in Veterinary Science, 2025)

Propentofylline

Used more commonly in Europe and Australia than in the US, propentofylline improves blood flow to the brain and has documented benefits for cognitive function and alertness in dogs with CDS. It is not FDA approved in the United States but is available in other markets and may be compounded by specialty pharmacies with a veterinary prescription. (AVMA Journal, 2025)

Melatonin

For dogs whose primary CDS symptom is sleep-wake cycle disruption, melatonin can be beneficial in restoring a more normal sleep pattern. It is available over the counter and is generally safe for dogs at appropriate doses — however dosage should be confirmed with your veterinarian, and products containing xylitol must be avoided as xylitol is toxic to dogs.

Anti-anxiety Medications

For dogs with significant CDS-related anxiety, anti-anxiety medications may be prescribed alongside primary CDS treatment. These do not address the underlying cognitive decline but can meaningfully improve quality of life for dogs whose anxiety symptoms are severe.

Senolytic and NAD+ Combinations

A 2024 randomized controlled clinical trial published in Scientific Reports demonstrated improved owner-assessed cognitive function in senior dogs receiving a senolytic and NAD+ precursor combination. This represents one of the more promising recent developments in CDS research, drawing on parallel work in human Alzheimer’s disease. The treatment is not yet widely available as a standard veterinary prescription but points toward where the field is heading. (Simon et al., 2024, Scientific Reports)

Butyrylcholinesterase Inhibitors

A 2021 PMC study of a novel butyrylcholinesterase inhibitor showed significant improvement in clinical ratings of cognitive ability and performance-based cognitive tests in dogs with moderate cognitive impairment compared to untreated controls. This class of drug parallels the cholinesterase inhibitor drugs used in human Alzheimer’s treatment and represents an active area of veterinary research. (PMC, BChEi Study, 2021)

Diet, Supplements, and Lifestyle

Diet

Diets enriched with antioxidants, omega-3 fatty acids, and medium-chain triglycerides have shown benefit in dogs with CDS. Hill’s Prescription Diet b/d and several other commercial senior diets are formulated with cognitive support in mind. Medium-chain triglycerides — a fat source derived from coconut oil — provide an alternative energy source for neurons that are struggling to metabolize glucose efficiently. Supplements

SAMe (S-adenosyl methionine) supports brain health and mood regulation and is available as a veterinary supplement under the brand name Novifit, among others. Phosphatidylserine has shown some benefit for cognitive function in older dogs. Apoaequorin (Neutricks) is a calcium-binding protein marketed for canine cognitive support with some clinical backing. All supplements should be discussed with your veterinarian before starting.

Environmental Enrichment

Mental stimulation slows cognitive decline. Puzzle feeders, scent work, new walking routes, gentle training sessions — keeping an older dog’s brain engaged has documented benefit for maintaining cognitive function. This is not about pushing a senior dog beyond their physical limits. It is about providing the low-level mental engagement that keeps neural pathways active.

Routine and Predictability

Dogs with CDS cope significantly better with consistent routine. Predictable feeding times, walk times, and sleep arrangements reduce the disorientation and anxiety that come with cognitive decline. Changes to the environment or routine should be minimized where possible. Nightlights in areas where the dog sleeps and moves at night can reduce nighttime disorientation.

What to Expect

CDS cannot be cured. Treatment options are aimed at slowing progression and managing symptoms rather than reversing the underlying neurological changes. (Embrace Pet Insurance, 2025)

What treatment can realistically achieve is meaningful. A dog whose sleep is stabilized, whose anxiety is managed, whose diet supports brain health, and whose environment is adapted to their declining navigation ability can have genuinely good quality of life for an extended period after diagnosis.

The progression of CDS varies between individual dogs. Some dogs decline slowly over several years. Others decline more rapidly. Early intervention — recognizing the symptoms and beginning treatment before the condition becomes severe — gives the best chance of maintaining quality of life for the longest possible time.

If you are seeing the signs, talk to your vet. The “just old age” conversation is worth pushing past. Your dog deserves the most accurate picture of what is happening and the best available response to it.
